Intertrochanteric Fractures of Femur Treated with a Proximal Femoral Nail

Abstract
Purpose: To evaluate the usefulness of a proximal femoral nail for the treatment of patients over 60 years of age with an intertrochanteric fracture of the femur.

Materials and Methods: Between January 2002 and November 2003, 42 patients with an intertrochanteric fracture of the femur were treated with a proximal femoral nail. The mean age of the patients was 72 years, with a mean follow-up duration of 12 months. The time for fracture union, the sliding distance, change in the femoral neck-shaft angle and walking ability were evaluated.

Results: The mean union time was 16 weeks. There was no significant difference in the union time between the stable and unstable fractures. The change in the femoral neck shaft angle was from 1300, on the immediately postoperative day, to 1270 at the last follow-up. The mean sliding distance of neck screw was 3.9mm. At the last follow-up, the mean Parker and Palmer mobility score was 8.2 points before the fracture and 7.3 points at the last follow up. A reoperation was necessary in 2 cases: excessive back-out of the neck screw, with gradual varus angulation, in 1 case, and additional wiring was performed in 1 case due to a poor reduction.

Conclusion: The proximal femoral nail is a useful device, in terms of the functional recovery of the patients, for the treatment of intertrochanteric fractures.
